As of May 1, 2024, our office has closed both the Friendswood and League City locations. Please see the instructions below for how to obtain your medical records.
Patients seen before June 26, 2023
Your records were in our old legacy system. These records have been transferred to Clary Document Management.
You may request copies of your records by filling out the form at medicalrecordcustodian.com/friendswood-gynecology.
Patients seen after June 26, 2023
Your records were in the new Privia system. These records can be requested by emailing medicalrecords@priviahealth.com.
The Privia system should have both the old records from before the June 26, 2023 migration and records after that date.
The records with Clary will only be those before June 26, 2023. There were a few random charts that did not completely migrate from the legacy system to Privia. If you feel that your chart is incomplete with Privia and you were seen in both systems, feel free to reach out to Clary using the link above to retrieve your records from the legacy system.
